Kaweah Delta asks patients to get in form
Kaweah Delta Health Care District is asking Tulare County residents to a New Year’s Resolution.
Tulare County residents can start the new year right by completing two healthcare forms to become active participants in their care.
One form to complete is a Medicine Form (Med Form). This form helps a person track the names and doses of medications that they are taking whether they are prescription or over-the-counter herbals or vitamins.
By filling out this form and sharing it with physicians and healthcare providers, patients become active participants in their care. This form also becomes important when someone visits an emergency department. That is a visit in which someone is not seeing their regular healthcare provider, but the form helps those treating a patient know which medications can be used to treat them and what medications must be avoided.

Advance HealthCare Directive
The second form is an advance healthcare directive. This is a document with special instructions, prepared in advance, which is designed to direct medical care for a person if they are unable to communicate.
It’s helpful because it provides physicians and family members with specific instructions about the medical care a person wishes to receive. It can also prevent treatment that a person would not choose for him/herself, decreasing feelings of helplessness and guilt for family members.
